General Information
Mental Health Rehabilitation Services is a compilation of services that serve individuals with serious and persistent mental illness utilizing a psychiatric rehabilitation framework.top
Mission Statement
To support individuals with serious and persistent mental illness to gain, or regain, the valued roles, skills and supports that they require so that they can be successful and satisfied in the environment(s) of their choice.Target Population
The population served by the Mental Health Rehabilitation Services are people with serious and persistent mental illness (SMI) who require intensive rehabilitation efforts to assist them in successfully reaching their goals. Clients can come from within the Mental Health Program or the broader community as an agency, GP or self-referral.top
Admission Criteria
Service recipients will typically have significant functional impairments secondary to their mental illness and will have a goal of improved functioning in at least one of: living, learning, social, working domains. There are no exclusionary or inclusionary criteria related to age or diagnosis; however, all participants should have a diagnosis of SMI. Assertive Community Treatment Team (ACTT)St. Josephs Mental Health and Addictions Program sponsors three Assertive Community Treatment Teams that serve different regions within our catchment. Assertive Community Treatment is an evidence-based program that provides intensive community-based treatment, rehabilitation and supports (including peer support) to the most seriously mentally ill for improved quality of life and independent living. ACT also supports individuals by assisting with medication, housing, finances, employment and life skills. Treatment is considered assertive because staff work with individuals in their home, place of work and other personal environments. Please contact:

Centralized Rehabilitation Services
This service is comprised of a variety of programs geared to assisting individuals with serious mental illness, both in and out of the hospital, to gain or regain valued roles through evidence-based approaches in psychiatric rehabilitation. Programs include:- Specialized rehabilitation group (e.g. Tobacco Addiction Recovery Program)
- Vocational assessment and counselling
- Supported employment and job readiness training
- Vocational computers
- Psycho-education/psycho-vocational assessment and consultation
- Recreation therapy (monthly rec calendar)
- Individualized supportive health and wellness counselling (e.g. weight management, medication management)
- Inspiration Place (clubhouse)

Homes for Special Care (HSC)
The HSC Program is a long-standing, province-wide, residential care home program for adult individuals with serious mental illness. The HSC Program, Central South, is a part of Mental Health Rehabilitation Services at West 5th Campus, and its leadership and management is the responsibility of West 5th Campus.There are 15 homes under the local HSC Program, located in urban and rural areas across Hamilton, Niagara, Haldimand, and Brant. At any given time, approximately 120-140 consumers reside in these HSC homes. Within the context of supervised, group home settings, home staff offer assistance with taking medication as well as with some activities of daily living.
Although HSC homes are independently owned, they are regulated and licensed by MOHLTC, and their operations and staffing are monitored by interdisciplinary clinical staff from the local HSC Program office. Additionally, HSC staff ensure that consumers receive therapeutic and other support services (e.g., counselling, vocational training, advocacy, referrals to various resources/facilities), carry out formal inspections of the homes, and make recommendations to MOHLTC for license renewals.
The Central South HSC Program also oversees two Skills Centres (one in Hamilton, the other in Vineland) which offer HSC consumers a range of rehabilitation opportunities to develop skills, gain meaningful experiences, and move on toward wellness and self-sufficiency. All Skills Centre programming (whether on or off-site, individual or group-based) are developed according to consumer-identified requests and/or needs, e.g., prevocational, avocational, life skills, recreational/social. Programming is designed and implemented to: (a) facilitate the development or acquisition of transferable skills within a safe, structured environment; (b) enable consumers to achieve their optimum level of functioning and well-being; (c) promote each individual's mainstream reintegration into his/her own community, wherever possible.
